To create a parent account and submit an online application:

1. Create an account
   • Easier to complete using a computer/laptop]
   • Create an account using the link above
   • Verify account in your email by clicking the link sent after set up
   • Use email as the username and password that was just created to log in
2. After logging in, at the top of the page you will find the "MEMBERSHIPS" tab
   • Select Joli Ann Leichtag as the Club location
   • Add your child as the contact for the membership and click next at the top
     • This will lead you to the membership where you will answer all of the questions for emergency contacts and household information
3. To complete the application, click next and "sign" the documents attached
   • When the application has been submitted it will ask for your email to send a confirmation of completion
